Responsible for accomplishing department objectives by managing staff; planning and evaluating department activities for Long Term Care. Responsible to plan and oversee the work of others.  The supervisor is responsible for planning and maintaining work systems, procedures and policies that enable and encourages the optimum performance of its people and other resources based on regulatory compliance within the Long Term Care services.

The Licensed Practical Nurse performs responsibilities that are supportive and/or restorative under the direction of a Registered Professional Nurse. Provides nursing care to an assigned group of patients, and administers medications and/or treatments upon demonstration of ability in accordance with policy of the Hospital.

Diagnose and treat patients with language, speech or hearing difficulty.  Identify treatment options, monitor progress and conduct testing.

Provide full social work coverage to include an assessment or patient’s psychosocial functioning, determine need for financial and medical assistance, provides supportive counseling, and arranges concrete services. Participate in counseling and determining the suitability for all treatment modalities.  Coordinate interdisciplinary consultation.

Assist in the performance of physical therapy treatment procedures and related duties under the supervision and direction of a licensed physical therapist.

Utilize the nursing process in the provision of direct patient care.  Responsible and accountable for prescribing, delegating, and coordinating all care provided to assigned patients.  Delivery of care is based on the standards of care and practices established by the Department of Nursing and other policies and procedures (hospital, nursing, multidisciplinary). Diagnose and treat human responses to actual and potential health problems through such services as case finding, health teaching, health counseling, and provision of care supportive or to restorative of life and well-being.  Supervise patient care provided by licensed practical nurses (LPNs), registered nurses on permit (RNP), nursing attendants, technicians, and supplemental staff.
